More Important Information

A card must be kept on file for each client. Cards will be charged the day of the session. Sessions must be paid for upfront. All sessions will be canceled if payment is not received before the session begins.  The fee for the initial intake assessment and treatment plan is $150 and $115 for each session thereafter.  Sliding scale costs will be on a case by case basis. I do accept insurance. Please see the list in FAQS.

Intake Session: $150 for 65 minutes

Individual Sessions: $115 for 50 minutes

Parent Coaching: $115 for 50 minutes

Family Sessions: 155 for 50 minutes

Phone Calls Lasting More Than 15 Minutes: $50 after every 15 minutes.

Empowered Princesses Therapy for Girls, LLC Documentation Requests:

Request for documentation to include letters, summary of services, progress summaries,  medical leave, court documentation, disability forms are assigned a fee of $150.00.

Please allow up to 14 business days to complete any request for documentation. Requests are to be submitted via secure email and or in person in office by completing documentation request form. Guardians must complete all required office paperwork.

Upon receipt of documentation request, an invoice will be provided for requested services, detailing fee and completion date.

Before any information is completed or released payment for services and any past due balances must be paid in full.

Good Faith Estimate:

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

 Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

  • You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

  • Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

  • If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

  • Make sure to save a copy or picture of your Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or call 800-985-3059.

Expedited Documentation Requests

Expedited request for a response or documentation within 1 business days is available for additional fee of $310.00.

Expedited request for a response or documentation within 3 business days is available for additional fee of $200.00.

 There will be no completion of any disability, progress summaries, or letters of support unless your therapist has met with you for at least 8-12 sessions.  We also will not complete any support documents or disability paperwork if we do not believe we can support it based on what you have presented at intake and during sessions.

 

Court Fee

As a reminder, that if you are involved in divorce or custody litigation, I do not make recommendations to the court concerning custody or parenting issues as the court can appoint these types of individuals. However, should you provide my name as a potential witness and I receive a subpoena to make an appearance at a deposition or trial, you will be financially responsible for my court fee. This fee is $1,000.00 per day (or any part thereof) due to my inability to be accessible to clients.
